Make an Appointment
You can use the form below to make a non-urgent appointment and we will
make every effort to accommodate you. Please be sure to include
your name, date of birth, I.D. number (if you have one), and select the
date and approximate time that would suit you. You should also indicate
what type of service you are requesting to enable us to book the necessary
staff. Blood tests are not carried out on Fridays.
Certain services / procedures are provided by certain doctors.
Please note that if you wish to make and urgent appointment, you should telephone the surgery directly on 052 21288 or 052 25236.
